When applying for nursing care leave, nursing care leave, or nursing care hours, one of the following documents is required to prove that you need constant care.
1.Certificate of long-term care required (nursing care required 2 or more)
2.Doctor's medical certificate (stating a condition that requires constant nursing care or can be presumed to be necessary)
3. Long-term care status determination check sheet
