This is not required for illnesses caused by pregnancy or childbirth. If you recover from an illness or injury other than pregnancy or childbirth while on maternity leave, please submit a "Certificate of Diagnosis Description" from your doctor at that time. When returning to work, you will be interviewed by an industrial physician, bringing the self-checklist and the previously submitted medical certificate. If there are no problems, you will be allowed to return to work on a trial basis or officially return to work. *If there are any concerns about the contents of the Certificate of Diagnosis Description, you may be asked to submit it again when returning to work.
