The following information is required on the medical certificate:
- Date of issue
- Your name
- Name of the injury or illness
- Period of absence required
-A statement indicating inability to work (such as "medical leave" or "home care").
If it is ambiguous whether you are unable to work, such as "outpatient treatment" or "under treatment," it may be returned.
*While employees on a long absence from work for illnesses related to pregnancy, their “maternity health care guidance item contact card” may be used as a substitute for a medical certificate.
Please submit either a PDF or image of the medical certificate. The original must be kept by the employee for one year upon returning to work, and must be submitted if requested by the company. For an overview on taking a leave of absence, please see People Connect.
